EMS Subscription Service Application
Parkview is offering an EMS subscription service for residents within the EMS service
area. For a yearly fee of $60 per household, subscribers are offered fully
covered ground transport. This includes high insurance deductibles and co-payment for
medically necessary emergency ambulance service.

What is Not Covered:
- Ambulance transports to and from physician office facilties, physical therapy, anc
pharmacies.
- Non-emergency transports which are not covered by insurance or third party coverage.
- Transports outside of Parkview Hospital's services area unless deemed medically
necessary and appropriate for treatment.
