recovering, disabled or chronically ill individuals who need medical treatment.
As an essential part of the community's healcare system, the Home Health staff works closely with your physician, the hospital and other community services to coordinate
all of your needs. Our goal is to promote individual independence while making
each patient's transition from a hospital or nursing home setting smooth and worry
free.

SERVICES PROVIDED
Physical Therapy
Physical therapy provides rehabilitation in the home for
a variety of diagnoses. Therapy is geared toward restoring function and proventing
disability from disease, injury or loss of a body part. Therapy is sensitive
to all the patient's needs. The therapist uses many different techniques to
strengthen, imporove circulation and improve mobility - allowing the patient to
return to their optimal level of function.
Occupational Therapy
According to the American Occupational Therapy Association,
occupational therapy is skilled treatment that helps individuals achieve independence
in all facets of their lives. It gives piople the "skills for the job of living"
necessary for independent and stisfying lives.
Speech Therapy
Speech therapy is a treatment program designed to help
patients regain their ability to swallow and communicate through speech. Therapists
work closely with a patient's physician to determine the most effective treatment
needed and may also work with family members or other individuals who may serve
as a patient's caregiver in order to ensure the patient receives an optimum level
of care in his or her everyday environment.

To Qualify, Patients Must:

Essentially be homebound,
requireing considerable effort and assistance to leave home (if receiving services
under Medicare).
- Be under a physicians's care.

Require skilled nursing services on an intermittent
basis, physical or speech therapy, or have a continued need for occupational therapy.
